Vision Benefit

Negotiated Vision Plan

Active employees with group MPS health care coverage automatically have MPS group vision plan benefits administered by National Vision Administrators (NVA).  The School Board pays the full premium for single and family coverage. 

 

Services Covered

Eye Exams - examinations by participating optometrists and opthalmologists are covered once each 12 months.

Eyeglass Frames - the plan allows a $35 frame allowance ($82 retail discount) towards the purchase of your frames. If the frame you choose exceeds this amount, you pay the difference. Frames are covered once per 12-month period for eligible plan participants.

Eyeglass Lenses - the plan allows payment of standard lenses (single vision, bifocal, trifocal or lenticular). If you choose a specialty lens, you pay the extra cost. For example, if you choose progressive multifocus lenses, your plan pays the amount for standard (lined) multifocal lenses. You would pay the additional cost for progressive lenses.

Eyeglass Lens Tints - solid tints are covered by the plan. Covered eyeglass tints do not include lenses with photochromic properties.

Contact Lens - the contact lens benefit is available in lieu of your frame and eyeglass lens benefit. The plan pays $100 towards the purchase of contact lenses; you pay any additional costs. This contact lens benefit is in addition to the eye examination benefit and is available once each 12-month period.

The plan pays for either eyeglasses or contact lenses, but not both during the same benefit period.  

 

Benefit Period - The 12-month benefit period is from date of service to date of service. For example, if you ordered glasses on July 1, you would be eligible for new glasses or contacts on July 1, one year later.

How to Use the Participating Provider Network

  • Choose a provider from the NVA Directory.  (You may obtain a paper copy by calling the NVA at 1-888-478-3722. Indicate that you are covered under the MPS' group plan.)
  • Call the provider directly to verify it is in the network.
  • Make your appointment.
  • Show your NVA identification card when you arrive for your appointment.
  • Fill your eyewear prescription at any provider location listed in the NVA directory.
  • Your provider will inform you of any additional expenses you owe.
  • Your provider will file the claim for you.
  • NVA will pay your provider directly for your covered expenses.

 

Services from a non-participating provider are not covered.
