If you get married, have a dependent older child return to eligibility, have a baby or adopt a child, you need to submit a new application form to MPS within 31 days (60 days for birth or adoption) to add your new dependent to your coverage. If you do not make a timely application, your new dependent will not be covered.
During the September open enrollment period, you can add dependents by submitting an MPS application form. Your added dependent's dental coverage will start on October 1 and health coverage will start on November 1.
You may obtain the appropriate application form from the MPS Department of Benefits and Insurance in Room 124 of Central Services.
Your spouse and children are covered – if you enroll them!
Even if you have family coverage, a new baby, for example, is not automatically covered.
To enroll a new dependent, you must file a new application form with MPS Benefits and Insurance Services.
We strongly recommend that you bring the form to Room 124 of the MPS central office and ask for a time-stamped photo copy – as proof of your timely application.

Your children are covered until the end of the month they reach age 25 if you provide more than 50% of their support for the calendar year.
Coverage is based strictly on financial dependence – whether they are college students or not.
Dependent grandchildren are not covered – regardless of their age and your level of support.
